The FDA labeling for efavirenz (Sustiva) and other non-nucleoside reverse transcriptase inhibitors (NNRTIs) includes specific warnings about reduced methadone concentrations when co-administered. The efavirenz prescribing information notes that increased methadone metabolism may occur, which can result in subtherapeutic methadone levels. Neither drug label explicitly contraindicates concurrent use, but both recommend therapeutic drug monitoring and potential dose adjustment.
The severity of this interaction is classified as moderate to significant because it directly affects the therapeutic efficacy of methadone maintenance therapy. For patients with HIV infection who also have opioid use disorder, this is a particularly important consideration, as managing both conditions simultaneously is complex and requires coordinated care between infectious disease specialists and addiction medicine providers.
Efavirenz is a non-nucleoside reverse transcriptase inhibitor that acts as a potent inducer of the cytochrome P450 enzyme system, particularly CYP3A4 and CYP2B6. These enzymes are responsible for metabolizing numerous medications, including methadone. When efavirenz is introduced, it increases the activity and expression of these metabolic pathways, causing methadone to be broken down more rapidly in the liver.
Methadone, a long-acting synthetic opioid agonist used for opioid maintenance therapy, depends heavily on hepatic metabolism via CYP3A4, CYP2B6, CYP2D6, and other pathways. Under normal circumstances, methadone has a long half-life (typically 24–36 hours), which allows once- or twice-daily dosing and stable opioid receptor occupancy. When efavirenz induces these metabolic enzymes, methadone's half-life shortens significantly, leading to faster elimination and lower trough concentrations.
This means that a patient whose methadone dose was previously adequate may experience declining plasma levels within days to weeks of starting efavirenz. The decline in methadone concentration translates to reduced mu-opioid receptor agonism, which can precipitate acute or protracted withdrawal symptoms: anxiety, insomnia, body aches, sweating, and intense cravings. In some cases, patients may return to illicit opioid use if their maintenance therapy becomes inadequate.
The interaction is bidirectional in terms of monitoring: efavirenz levels are not significantly affected by methadone, so the concern is primarily about maintaining adequate methadone efficacy, not antiretroviral drug exposure.

Marcus is a 45-year-old man with a 20-year history of heroin use disorder who has been stable on methadone maintenance therapy at a community health center for 8 years. His current methadone dose is 70 mg once daily, and he has not used illicit opioids in 7 years. He recently tested positive for HIV with a CD4 count of 180 cells/mm³, and his infectious disease specialist initiates a first-line antiretroviral regimen including efavirenz, emtricitabine, and tenofovir (EFV/FTC/TDF).
Two weeks after starting the antiretroviral regimen, Marcus reports to his methadone clinic with complaints of insomnia, mild anxiety, and body aches. His counselor initially assumes these are early signs of an opportunistic infection or HIV-related illness, but Marcus clarifies that these symptoms feel exactly like the withdrawal he experienced years ago when he accidentally missed methadone doses. His methadone provider, aware of the potential efavirenz-methadone interaction, orders a methadone level and finds it has dropped from a baseline of 200 ng/mL to approximately 120 ng/mL—a 40% reduction.
Marcus's methadone dose is increased to 100 mg daily, and his symptoms resolve within 3 days. The clinic and his HIV provider establish a protocol: Marcus will have methadone levels drawn at weeks 2, 4, 8, and 12 after initiating efavirenz, with dose adjustments as needed. By week 8, Marcus stabilizes at a dose of 95 mg daily, and his methadone levels remain in the therapeutic range (100–200 ng/mL). His CD4 count rises appropriately with antiretroviral therapy, and he remains engaged in both HIV care and addiction services.
Janice is a 38-year-old woman with HIV infection (on an unrelated antiretroviral regimen) who was recently switched to efavirenz-based therapy due to virologic failure on her prior regimen. She has been on methadone maintenance for 5 years at a dose of 50 mg daily and has not used opioids since starting treatment. Her methadone is managed at a private clinic that does not routinely communicate with her HIV specialists.
Three weeks after starting efavirenz (in combination with other agents), Janice experiences acute onset of severe anxiety, insomnia, muscle aches, and intense cravings for opioids. She contacts her methadone clinic and is told that her dose cannot be increased without clinic director approval and that she "should talk to her HIV doctor." She becomes increasingly distressed and, out of desperation, contacts a former dealer and uses heroin for the first time in 5 years. This leads to overdose, hospitalization, and ultimately a lapse in both HIV and addiction treatment adherence.
Post-event review reveals that Janice's methadone level had fallen to 45 ng/mL (from a baseline of approximately 100 ng/mL)—a 55% reduction. Had her methadone and HIV providers communicated and anticipated this interaction, her dose could have been proactively increased or therapeutic drug monitoring scheduled immediately after efavirenz initiation.

Additionally, certain drugs inhibit methadone metabolism (such as fluconazole or azithromycin), which could potentially offset the inducing effect of efavirenz. For patients on efavirenz and fluconazole concurrently, this becomes particularly complex. Similarly, methadone and fluconazole interaction can increase methadone levels, which could partially counterbalance efavirenz-induced reduction.
