Moexipril Hydrochloride

Do not coadminister aliskiren with moexipril hydrochloride in patients with diabetes. Avoid use in patients with renal impairment due to increased risks of hypotension, hyperkalemia, and renal dysfunction.

Dual RAS blockade is associated with increased risks of hypotension, hyperkalemia, and changes in renal function including acute renal failure compared to monotherapy.

ACE inhibitors can increase serum lithium levels and symptoms of lithium toxicity. These drugs should be coadministered with caution and frequent monitoring of serum lithium levels is recommended.

COX-2 inhibitors may result in deterioration of renal function, including acute renal failure, especially in elderly, volume-depleted, or renally compromised patients. Antihypertensive effect may be attenuated.

NSAIDs may result in deterioration of renal function, including acute renal failure, especially in elderly, volume-depleted, or renally compromised patients. NSAIDs may also attenuate antihypertensive effect.

Injectable gold has been associated with rare nitritoid reactions (facial flushing, nausea, vomiting, hypotension) when combined with moexipril and other ACE inhibitors.

Potassium-sparing diuretic may increase risk of hyperkalemia when used concomitantly with moexipril. Requires caution and monitoring of serum potassium levels.

Excessive reductions in blood pressure may occur when ACE inhibitors are started in patients on diuretic therapy. Risk can be minimized by discontinuing diuretics or cautiously increasing salt intake before initiation.

Concomitant use with moexipril can increase risk of hyperkalemia. Should be given with caution and patient's serum potassium should be monitored.

Potassium-sparing diuretic may increase risk of hyperkalemia when used concomitantly with moexipril. Requires caution and monitoring of serum potassium levels.

Potassium-sparing diuretic may increase risk of hyperkalemia when used concomitantly with moexipril. Requires caution and monitoring of serum potassium levels.

Interaction studies failed to identify any clinically important effect on serum concentrations or anticoagulant effect of warfarin.

Other Agents No clinically important pharmacokinetic interactions occurred when moexipril hydrochloride was administered concomitantly with hydrochlorothiazide, digoxin, or cimetidine.

The possibility of hypotensive effects with moexipril hydrochloride can be minimized by discontinuing diuretic therapy for several days or cautiously increasing salt intake before initiation of treatment with moexipril hydrochloride. If this is not possible, the starting dose of moexipril should be reduced (see WARNINGS and DOSAGE AND ADMINISTRATION ).
