Doxazosin

Alpha-1 blocker; concomitant administration may lead to symptomatic hypotension. Combination is not recommended.

Concomitant administration can result in additive blood pressure lowering effects and symptomatic hypotension.

Alpha-adrenergic blocking agent can antagonize the effects of midodrine hydrochloride.

Clinical pharmacology studies conducted with coadministration of tadalafil with doxazosin. Additive blood-pressure-lowering effects anticipated.

Cimetidine increases doxazosin AUC by 10%. Clinical significance of this increase is unknown; monitor for increased effects.

7 DRUG INTERACTIONS Strong cytochrome P450 (CYP) 3A inhibitors may increase exposure to doxazosin and increased risk of hypotension. ( 7.1 ) Concomitant administration of doxazosin tablets with a phosphodiesterase-5 (PDE-5) inhibitor can result in additive blood pressure lowering effects and symptomatic hypotension. ( 7.2 ) 7.1 CYP3A Inhibitors In vitro studies suggest that doxazosin is a substrate of CYP3A4.

Postural hypotension reported in patients taking both beta-blockers and doxazosin.

Alpha-blocker causing additive blood pressure lowering effects with sildenafil, with reports of symptomatic postural hypotension including dizziness and light-headedness.

Alpha-blocker causing additive blood pressure lowering effects; mean additional reductions of 7/7 to 11/4 mmHg observed with reports of symptomatic postural hypotension.

In vitro data indicate doxazosin has no effect on protein binding of digoxin.

In vitro data indicate doxazosin has no effect on protein binding of indomethacin.

In vitro data indicate doxazosin has no effect on protein binding of phenytoin.

In vitro data indicate doxazosin has no effect on protein binding of warfarin.

Both are highly protein-bound and hepatically metabolized. No definitive controlled clinical experience on concomitant use of alpha 1 inhibitors and 5-alpha reductase inhibitors.

Central Alpha1-Blockers Doxazosin: Healthy volunteers participating in a multiple dose doxazosin-nifedipine interaction study received 2 mg doxazosin q.d. Co-administration of nifedipine resulted in a decrease in AUC and C max of doxazosin to 83% and 86% of the values in the absence of nifedipine, respectively. In the presence of doxazosin, AUC and C max of nifedipine were increased by factors of 1.13 and 1.23, respectively.
