Neomycin

Concurrent and/or sequential use should be avoided due to potential neurotoxicity and nephrotoxicity.

Neomycin The concomitant use of sorafenib tablets with neomycin decreased the mean AUC of sorafenib, which may decrease the antitumor activity. Avoid concomitant use of sorafenib tablets with neomycin.

If coadministration cannot be avoided, monitor closely for methotrexate adverse reactions when coadministered with: Oral antibiotics (including neomycin) Antifolate drugs (e.g., dapsone, pemetrexed, pyrimethamine and sulfonamides) Oral or intravenous penicillin or sulfonamide antibiotics Aspirin and other nonsteroidal anti- inflammatory drugs Hepatotoxic products Highly protein-bound drugs (e.g., oral anticoagulants, phenytoin, salicylates, sulfonamides, sulfonylureas, and tetracyclines) Proton pump inhibitors Weak acids (e.g., salicylates) Nephrotoxic products Probenecid Nitrous Oxide Coadministration of methotrexate with nitrous oxide anesthesia potentiates the effect of methotrexate on folate-dependent metabolic pathways, which may increase the risk of severe methotrexate adverse reactions.

Neomycin has nondepolarizing neuromuscular blocking action; neostigmine dose adjustments may be required to reverse neuromuscular block.

Parenteral administration may intensify or produce neuromuscular block and paralysis when used with pyridostigmine.

Neomycin may intensify or produce neuromuscular block on its own and can cause unexpected prolongation of vecuronium-induced neuromuscular blockade.

Decreases digoxin concentrations. Requires monitoring and dose adjustment.

Elimination of colonic bacteria by neomycin may interfere with lactulose degradation and prevent acidification of colonic contents. Close monitoring recommended during concomitant use.

Drug Interactions Caution should be taken in concurrent or serial use of other neurotoxic and/or nephrotoxic drugs because of possible enhancement of the nephrotoxicity and/or ototoxicity of neomycin (see boxed WARNINGS ). Caution should also be taken in concurrent or serial use of other amino-glycosides and polymyxins because they may enhance neomycin’s nephrotoxicity and/or ototoxicity and potentiate neomycin sulfate’s neuromuscular blocking effects. Oral neomycin inhibits the gastrointestinal absorption of penicillin V, oral vitamin B-12, methotrexate and 5-fluorouracil.
