Concomitant use results in reduction of valproic acid concentrations below therapeutic range, increasing risk of breakthrough seizures. Generally not recommended.
Mechanism
Carbapenems may inhibit hydrolysis of valproic acid's glucuronide metabolite back to valproic acid
