At normal doses, simvastatin works by inhibiting HMG-CoA reductase, an enzyme crucial for cholesterol synthesis. But when clarithromycin prevents simvastatin from being eliminated, the drug accumulates to concentrations far above those studied in clinical trials. By day 3 of clarithromycin therapy, the patient develops severe muscle pain (myalgia) in both thighs and calves. He attributes it to overuse and continues taking both medications. By day 5, he notices dark urine and reduced urine output—signs of acute kidney injury from muscle breakdown products (myoglobin) overwhelming the kidneys. He is admitted to the hospital, where laboratory tests reveal:
He is diagnosed with statin-induced rhabdomyolysis. Treatment requires discontinuation of both simvastatin and clarithromycin, aggressive IV hydration, and intensive monitoring. His kidney function gradually recovers over 10 days, but the experience was serious and avoidable.
The lesson: Even in an older adult on a moderate simvastatin dose, the combination caused severe muscle toxicity. The contraindication exists for exactly this reason.
