When combined with warfarin's inhibition of vitamin K-dependent clotting factors (II, VII, IX, X), the result is dual anticoagulant action—warfarin reduces clotting factor synthesis while ketorolac impairs platelet plug formation. This combination substantially raises bleeding risk.
NSAIDs reduce protective prostaglandins in the gastric mucosa and inhibit platelet aggregation in mucosal blood vessels. This increases risk of ulceration and bleeding. Warfarin amplifies this risk because even small GI bleeds may not clot effectively. The combination of anticoagulation and NSAID-induced mucosal injury creates a perfect storm for significant GI hemorrhage.
Ketorolac's potency as an NSAID is relevant here: it is one of the stronger NSAIDs on a milligram-for-milligram basis, which is why it is indicated only for short-term acute pain. Its intensity amplifies all three interaction mechanisms above.
Age and renal function: Patients over 65 with reduced renal clearance face heightened risk because ketorolac undergoes significant renal elimination. Reduced clearance prolongs drug exposure and amplifies the interaction. Warfarin's metabolism is hepatic, but renal disease often correlates with hepatic dysfunction in elderly patients.
History of GI bleeding: Any patient with prior peptic ulcer disease, gastritis, or GI bleeding is at substantially elevated risk. The combination of anticoagulation plus NSAID is contraindicated in this population even as a single dose.
Concurrent antiplatelet therapy: Patients on warfarin plus aspirin or clopidogrel who are then given ketorolac face triple anticoagulant/antiplatelet effect—a particularly dangerous scenario.
High warfarin intensity: Patients whose target INR is 2.5–3.5 (mechanical heart valve) are at higher baseline bleeding risk than those with INR target 2–3 (atrial fibrillation). Ketorolac-induced INR elevation is more clinically significant in high-intensity patients.
Liver disease or alcohol use: Patients with hepatic impairment have reduced warfarin metabolism, making them more INR-labile. Chronic alcohol use compounds this.
Acute kidney injury: Any episode of dehydration or acute renal injury during ketorolac use will reduce both drug's clearance, intensifying the interaction.
A 72-year-old man is on warfarin for atrial fibrillation (INR goal 2–3, currently therapeutic at 2.4) with stable dosing for 6 months. He undergoes a dental extraction under local anesthesia and experiences significant post-operative pain. The dentist prescribes ketorolac 30 mg IV for 5 days, unaware of the warfarin. After 48 hours, the patient notices increased bleeding from the extraction site that doesn't stop after 15 minutes of pressure. He calls his anticoagulation clinic and his INR is checked stat: it has risen to 4.2. He is counseled to discontinue ketorolac, increase oral vitamin K, and recheck INR in 48 hours.
