Aliskiren

Concomitant use of aliskiren with ARBs in diabetic patients is contraindicated. In non-diabetics, avoid combined use particularly with CrCl <60 mL/min due to increased risk of hypotension, hyperkalemia, and renal failure.

Contraindicated with azilsartan in patients with diabetes. Avoid in patients with renal impairment (GFR <60 mL/min). Dual RAS blockade increases risk of hypotension, hyperkalemia, and acute renal failure.

Dual Blockade of the Renin-Angiotensin-Aldosterone System (RAAS): The concomitant use of aliskiren with other agents acting on the RAAS such as ACEIs or ARBs is associated with an increased risk of hypotension, hyperkalemia, and changes in renal function (including acute renal failure) compared to monotherapy.

Furosemide: Oral coadministration of aliskiren and furosemide reduced exposure to furosemide. Monitor diuretic effects when furosemide is coadministered with aliskiren.
